A visit, not a subscription
A pellet is placed in clinic and releases estradiol over roughly three months. The charge belongs to the visit. There is no monthly billing event to compare with a subscription plan, which is why the monthly figure has to be derived rather than read.
The $450 in the headline is a worked example, not a program in this dataset: no record here prices a pellet. Where a record does hold a per-visit charge, the arithmetic is the same and the figure is labeled derived.
If the record also carries an unpriced required lab panel, the all-in figure is withheld. That is not a criticism of the program; it is a statement that the program has not published the number.
What the derived figure is good for
It lets a per-visit charge sit in the same column as a 28-day cycle without pretending they are the same kind of payment. Used that way it is useful. Quoted as though it were a monthly bill, it misleads.
