HRT price rows, one provider per product
A row is a charge, its cadence and its source. The derived monthly figure is not stored, because it is arithmetic rather than evidence: anyone can recompute it from the row.
Field rules
Charges are stored as billed. Where a provider bills every 28 days, a year holds 13.04 cycles rather than 12.
charge_usd
Stored as billed. A blank is null; a genuine zero is 0.
billing_cadence
One of calendar-month, fixed-days or not-established.
billing_days
Required when the cadence is fixed-days. Never borrowed from the supply period.
membership_fee_monthly_usd
A number, an explicit 0, or blank. Blank is not 0.
membership_included_in_charge
True when the membership is already inside the recurring charge and must not be added twice.
required_fees
Each fee is bucketed confirmed, unresolved or optional. Unresolved withholds the all-in figure.
state_list
all means fifty states; otherwise a semicolon-separated list.
source_url + check_date
Names where the figure came from and when it was read. A blank means no source stated it; it is never 0.

Two lists gave $69 and $79 for what looked like the same monthly charge.
Neither figure appears on the provider's own pages. The $199 on the site is compounded GLP-1 weight-loss medication, not HRT. Both listings were withdrawn, and the HRT charge is now recorded as not published rather than filled from a third party.

One list stated $54 all-in; another stated about $120 for estrogen plus progesterone.
Resolved on re-read: Winona prices each formulation separately. The combined cream is $89 and the estradiol patch $149, both quoted as 'from' prices and recorded as two offers. A 30-day supply is processed every 28 days, so each charge recurs 13.04 times a year, giving $96.75 and $161.97 a month. Tablets are published as a $45 to $54 range and are not used as a rate.

Dropped as a starting price. The patch ships quarterly at $224.97 for a 12-week supply with progesterone included, which is $81.52 a month, not $74.99. The $49.95 consultation is held as a one-time fee outside the monthly figure.

The file gave $20, which was read as a monthly total.
Resolved on re-read: $20 is the oral progesterone add-on, not a monthly total. The Estrogen Cream is $92 per two-month supply, the pill $70 and the patch $75, each recorded separately. A cream carrying progesterone is documented as a Trio formula, but no charge for it appears on the pricing page read, so no price is recorded for it.
